• Click on it and the id retrieves page opens. Enter the following data to retrieve id:

1. First name
2. Last name
3. Birthdate
4. Last 4 digits of SSN

• Click on the ‘Search’ button and you can find the ID.

Retrieve Email Address

• If you have forgotten email address linked to your account open the login homepage.
• Next click on the ‘Retrieve Email Address’ button below the login widget.
• As the retrieved page opens enter the following search criteria:

1. Choose account type from the Search by menu
2. Term
3. First name
4. Last name

• Hit the search button and you can find the email address
