- If your qualifying purchase was an Online Order, click the specific link next to ‘weis2GO Online’ at the top of the survey entry page.
- Enter the required details i.e. Location or Store Number, Reward Card Number, Date and Time of Visit from your Weis Markets receipt in their respective fields on the page. (Take help of the sample receipt and Weis Rewards Card images on the left-hand side of the entry fields if you have a problem to locate the information).
- Click on the “Start” button to begin the survey.
- Answer all the survey questions; give your feedback and tell them about your experience with Weis.
- Finish the survey completing all the required fields and provide the details you will be asked for.
If your Weis Markets Survey Receipt contains a Phone Survey Invitation, you can also call that number and take the survey providing all the necessary information and following the voice prompts.
